I want to congratulate you for investing in yourself, your health and your life!  Saying YES to yourself is one of the best gifts you can give yourself and I am thrilled to be able to support you in reaching your goals.

Before we begin, I invite you to take a few moments to fill out this confidential health history and welcome packet.  

This will help you get clear on your intentions and goals and help me get to know you better.

    By clicking "I Agree," entering your credit card information, or otherwise enrolling, electronically, verbally, or otherwise, in this program, you ("Client") are entering into a legally binding agreement with Corpo Vita Studio, INC and a New York Corporation ("Company"), and Tracy Campoli according to the following terms and conditions:

The Creators, producers and distributors of this program disclaim any liability in connection with the exercises and advice herein.
    You have been informed of the need for medical approval before joining in this fitness exercise, nutrition and training program which Tracy Campoli will design.
    You are voluntarily choosing to participate in a physical exercise program and health coaching program
    You agree that any information, instruction or advice obtained from Tracy Campoli may NOT be used as a substitute for your doctor’s advice or treatment.
    You agree that any information, instruction or advice obtained from Tracy Campoli will be used at your own risk.
    You agree to release and discharge Tracy Campoli from any and all responsibilities or liabilities from injury arising from your participation in any exercise program designed by Tracy Campoli on your behalf.
    You agree to compensate Company according to the payment schedule agreed upon via website and/or email. Company shall charge a 5% late payment fee to any payments not made in a timely manner.
    Upon execution of this Agreement, Client shall be responsible for the full extent of the Fee. If client cancels attendance at the Program for any Client will receive no refund, with the exception of a medical condition with a doctors note, and agreement to be interviewed by Tracy Campoli or one of her team members before any medical refund is processed.
    Upon execution by clicking “I agree,” the Parties agree that any individual, associate, and/or assign shall be bound by the terms of THIS AGREEMENT. A facsimile, electronic, or e-mailed executed copy of this Agreement, with a written or electronic signature, shall constitute a legal and binding instrument with the same effect as an originally signed copy.
    Permission is given for the downloading and storage of one or more of these pages and videos for the purpose of viewing on a personal computer.
